"Escúchame" is in the imperative mood, that is to say, it's a command and it means "listen to me".
"Me escucha" is in the indicative mood and it doesn't mean anything per se, It translates to "you (formal)/he/she/it listens to me"

Escuchame lo puedes asociar al verbo "escuchar" donde haces uso del pronombre de objeto directo hacía tu persona y le das a entender alguien que debe prestarte atención.
El segundo es una forma de preguntar si te están prestando atención o si pueden oír lo que dices.
